QI REGISTRY

1. Take Our Survey

Once you submit the survey, you’ll receive the following Program Materials for your review .

          1. Program Guidelines
          2. Centralized IRB Exemption for SPP (received by SSSF)
          3. Data Collection Forms: for for Idiopathic Scoliosis, Neuro and/or Spondy modules, as requested.
            1. Pre-Op/Op, Post-Op/Peri-Op, Comps w/in 90 days
          4. Registry Participation Agreement
            1. With Business Associate Agreement & Data Use Agreement provisions included

2. Review SPP Guidelines (& Submit to Your IRB, if required.)

SSSF has obtained a centralized IRB exemption waiving oversight: quality improvement projects do not meet the definition of research based on federal regulations* 45 CFR 46. Utilize our centralized IRB exemption or submit to your local IRB if required by your institution.

*Regulations may differ for institutions outside of the U.S.

3. Establish a Data Entry Plan

The required data-sets for SPP were developed to be feasible by a surgeon independently, regardless of the availability of administrative support, however, sites will need to designate an individual or individuals who will be responsible for entering data for the registry. Additionally, sites should consider whether they will include optional PHI variables or strictly deidentified patient data.

4. Execute Your RPA

Participants are required to sign and return a Registry Participation Agreement (RPA). If a participant opts to submit PHI, the RPA will include supplemental Business Associate (BAA) and Data Use (DUA) Agreement provisions. Upon receipt, you will be emailed a fully-executed agreement from SSSF.

5. Enter Data!

You can start entering your data and start receiving real-time reporting thru your Dashboard Reports .

How To Join

RESEARCH REGISTRY

1. Review the Program Brochure

2. Submit Your IRB

Submit to your IRB based on how you would like to utilize the database.

3. Execute your Database User Program Services Agreement.

Read the subscription terms and provide an executed agreement to mlee@ssshsg.org.

4. Provide billing information and submit payment.

See Exhibit A of the Database User Program Services Agreement (p.6-7).

5. Enter Data!
